Department of Public Services

The following forms are available on-line for these DPS Divisions;  DPS Administration, Highway & Fleet, Water & Sewer, and Environmental Services (Parks & Recreation, Recycling & Solid Waste, and Tree Warden).  These may be completed on-line and printed, however, you are not able to electronically submit the forms and fees at this time.  We hope to offer that service in the not-to-distant future.  More information about the various forms are generally available from our FAQ's page.

DPS Administration
Application for setting up a new owner on the utility account or for changing the existing owners mailing address.
DPS Administration
Application for requesting an abatement or adjustment on your utility bill. (i.e.  error/miscalculation, vacant property, complete pool refill, major water leak, or late filing for a discount.)  
Highway
Application for a permit for performing work in the public way including roads, sidewalks, and unpaved shoulders.  Fee schedule is included here.
Highway
Application for a permit required by State law for ALL excavations, public and private, more than three feet deep.  Excavations performed wholly on private property are handled through the Building Department.

Application for a new traffic warning or control sign to be placed in the public way.

Policy and application for compensation for mailboxes damaged by snow removal operations.
Parks & Recreation
Application and permit for reserving time on public parks and athletic fields. Fee schedule is included here.
Parks & Recreation
Application and permit required for use of the Garnsey Dog Park. All dogs require a permit tag to use the park.
Sewer
Application for a permit for repairing, replacing, or installing a sanitary sewer service or main on either public or private property.   If the excavation includes any digging in the public way, a Street Opening Permit and Trench Permit will also be needed.  Fee schedule is included here.
Recycling & Solid Waste
Application special one day entry to the Recycling Center for vehicles not normally permited.
Water
Application for a permit for repairing, replacing, or installing a water service or main on either public or private property.   If the excavation includes any digging in the public way, a Street Opening Permit and Trench Permit will also be needed.  Fee schedule is included here.
Water/Sewer
Application for a license to work on the Town of Medway's Water and Sewer system for a calendar year (installations, repairs, replacements, and cut/cap's).  Specific insurances, bonds, and equipment licenses are required.
