How do I obtain a copy of a birth, death, or marriage certificate?
Certified copies of birth, death, and marriage records are available from the Town Clerk's Office.  Copies of vital records may be obtained in person or by mail.  Please call and order, cash and checks are accepted for payment.  The cost is $10.00.

Who and how can one register to vote?
Any person who is a Massachusetts resident, a United States citizen and will be eighteen years old as of the next election, is eligible to register to vote.  When you sign the registration form, you must attest, under the pains and penalties of perjury, that you are legally eligible to register and that the information you provide is accurate and truthfull.
You will remain on the list of voters so long as you vote regularly and complete and return the annual town census.  If yo move to another city or town, you must re-register in your new community.

How do I obtain a dog license?
All dog owners must obtain a license for their dog(s) each year.  Please note these important dates:
  • January 1st dog licenses available.  Cost is $11.00.
  • June 15th dog owners after this date will be charged a $10.00 late fee.
You may obtain a dog license at the Town Clerk's office during regular office hours.  You may also obtain license by mail.  All dog owners must submit proof of up to date rabies vaccination.

The fee schedule is as follows:
  • Neutered and spayed $11.00.
  • Male and Female $16.00.
How do I obtain a marriage license?
Persons wishing to marry in Massachusetts must file their intentions to marry with any Massachusetts City or Town Clerk in person (except for in cases where one person is in the military or incarcerated) at least three days before the ceremony.
In Medway, the cost of filing marriage intentions is $30.00 and $10.00 for a certified copy.  The license is valid for sixty days, and may be used for aceremony conducted in any Massachusetts city or town.

What is the absentee voting process and how do I obtain an Absentee ballot?
Applications for absentee ballots must be received by the Town Clerk before 12:00 noon the day before an election.  (A voter who is admitted to a health care facility after noon of the fifthe day before an election may apply for a ballot up until the close of the polls and may request to have the ballot delivered.)
If the voter submits the application in person, he may obtain the ballot and bote over-the-counter.  The voter should first check with the office ot see if the ballots are available.  If the ballot is not available, it will be sent to the voter by mail.  When the application is submittted in person by a family memeber, the ballot must be sent by mail to the voter.

What is the voting process for the town?
The polls are open in Medway from 7:00 am - 8:00 pm for state and federal elections, 8:00 am - 8:00 pm for town elections.  There are four precincts, everyone votes at the Medway Middle School on Holliston Street.
Elections are held:
  • 1st Monday in May for Town Election
  • 2nd Monday in May for Annual Town Meeting
How do I obtain a marriage certificate?
The clerk's office will have marriage records of any couple who filed their marriage intentions in Medway.  A certified copy of a marriage certificate is available for $10.00.

How do I obtain birth certificates?
The Clerk's office will have birth records of anyone whose parents lived in Medway at the time of the birth.  A certified copy of birth is $10.00.  A record can also be obtained where the birth occurred.

How do I obtain a business certificate (D/B/A)?
Whenever you are conducting a business under a name other thatn your own or the corporation's you must obtain a Business Certificate from the Town Clerk in the town where the business is located.  The business certificate is maintained in a public file and allows consumers to identify and locate the owner.  The filing fee is $40.00 and is good for four years.

How do I make a request for public information?
The public records law allows ten (10) days to fulfill a request.  Requests may be oral or written.

How do I obtain death certificates?
The clerk's office will have death records of any death that occurred in Medway and copies of records of Medway residents whose death occurred within the State of Massachusetts.  The fee for a certified copy of death is $10.00.

How do I obtain burial permits?
Burial permits are obtained at the Town Clerk's office during regular hours.

How do I obtain raffle permits?
Any orgainization holding a raffles in the Town of Medway must apply for a raffle permit.  Only non profit organizations are eligible.  The cost is $10.00 and is good for a year.

How much notice must be given for a governmental public meeting?
All meetings of any governmental body must be posted 48 hours beforehand.  All postings are posted outside the Town Clerk's office.

How do I pay a Non-Criminal disposition (ticket)?
Tickets written by the dog officier, Board of Health or Building Inspector are paid at the Town Clerk's Office.  All tickets and payments are recorded.

What if I'm a permanently disabled voter?
A voter who is permanently disabled need not submit an absentee ballot for every election.  If such voter submits a note from a registerd physician indicating that the voter is permanently disabled, the Town Clerk will send a ballot for all elections.
